What Your Gut Has Been Trying to Tell You

Your jeans still fit. Or maybe they don't.

Either way, something has shifted.

Bloating that wasn't there before. Foods you've eaten for years that suddenly don't agree with you. Digestion that feels unpredictable, sluggish, or just… different. A sense that your body has changed the rules without telling you.

Most women who describe this to me have already been told it is stress, or age, or something they should simply eat around. Very few have been told that the digestive system is hormonally sensitive — and that what is happening in the gut during perimenopause and menopause is part of the same transition as the hot flushes and the broken sleep.

It is worth saying plainly: the change does not stop when periods do. The body continues adapting well into the postmenopausal years, and this is one of the least researched parts of menopause medicine.

The conversation between your hormones and your gut

There is a name for this: the estrobolome.

It describes the community of gut bacteria involved in how oestrogen is processed and recirculated in the body. Oestrogen is metabolised in the liver and sent into the gut for elimination. Certain gut bacteria produce an enzyme that can reactivate some of it, allowing it to be reabsorbed rather than excreted.

In other words, the microbiome has a hand in how much oestrogen and what type of oestrogen stays in circulation.

The relationship runs both ways. Your hormones shape the gut environment, and the gut helps shape your hormone exposure. This is not a fringe idea — it is an area of active research, and one of the more interesting frontiers in women's health.

It also explains something many women recognise long before midlife: digestion that shifts across the menstrual cycle. Constipation in one phase, looseness in another, appetite that moves with the weeks. That is the same system, responding to the same signals, on a smaller scale.

What changes in perimenopause and Menopause

When oestrogen becomes erratic in perimenopause and then declines in Menopause, that existing sensitivity becomes louder.

Several things are happening at once:

Microbial diversity shifts. Oestrogen influences the composition of the gut microbiome, and lower or fluctuating levels are associated with reduced diversity — which matters, because diversity is one of the more reliable markers of a resilient gut.

Motility changes. Oestrogen and progesterone both affect how quickly things move through the digestive tract. As they fluctuate, transit time becomes less predictable, which is often felt as bloating, irregularity, or a sense of fullness that lingers.

The gut barrier becomes more permeable. Oestrogen has a role in maintaining the integrity of the intestinal lining. When it drops, that barrier can become less tight, which is one proposed mechanism behind the low-grade inflammation that tends to increase across the menopause transition.

Inflammation and immunity respond. Roughly seventy per cent of immune tissue sits around the gut. Changes in the microbial environment do not stay local — they are felt in mood, energy, joint comfort, and metabolic function.

Why some women feel this more than others

The complete answer is not clear, but the observation is that this is felt most acutely by women with autonomic dysregulation.

The gut and the nervous system are in constant communication through the vagus nerve — the gut–brain axis. Digestion is largely a parasympathetic function; it happens when the body is in a settled state. When the nervous system is running sympathetically dominant — through chronic stress, poor sleep, or an underlying tendency towards dysregulation — digestion is deprioritised.

Add a hormonal transition that is itself destabilising to the autonomic nervous system, and the two systems amplify each other. The gut becomes more reactive. The nervous system reads that reactivity as another signal of threat.

This is why two women of the same age, with similar diets, can have entirely different experiences of the same transition. It is not willpower, and it is not imagination. It is physiology.

Where understanding takes you

Here is the part worth holding onto.

The estrobolome is not fixed. The microbiome is one of the most responsive systems in the body — it changes with what you eat, how you sleep, how you move, and the state of your nervous system. It is influenced daily, whether or not you are paying attention to it.

And the inputs are broader than food alone. It is not only what you eat. It is when you eat, how you eat, why you are eating, and who you are eating with. Eating in a rushed, sympathetically activated state is a different physiological event than eating slowly at a table with people you like — even if the plate is identical.

Your gut is not failing you. It is responding to a shifting internal landscape.

And that conversation is one you can take part in.

Understanding it is where that begins — because every aspect of how we live is worth reviewing when it comes to digestion.
